What consumer health data is
Consumer health data is information that is linked or reasonably linkable to you and that reflects your past, present, or future physical or mental health, including the health-related services you seek or receive.
What we collect, and why
Through our website we may collect a limited amount of information that could be considered consumer health data—such as your name and contact details and any description you provide of the support you are looking for when you submit an inquiry or join our mailing list. We collect this only to respond to you, coordinate care, and send communications you have requested.
Where this information comes from
We collect this information directly from you when you fill out a form, send a message, or subscribe to our list. We do not purchase consumer health data or obtain it from data brokers.
What we share, and with whom
We do not sell consumer health data, and we will not share it for advertising. We share it only with the service providers that help us operate the practice and website, and only as needed for them to perform their role:
- Our practice-management and scheduling platform
- Our website host
- Our email and newsletter service
We may also disclose information if required by law. We do not use geofencing around any location that provides in-person health care to identify, track, or collect data from consumers.
Your rights
If you are a Washington resident, you have the right to:
- Confirm whether we collect, share, or sell your consumer health data, and access that data
- Withdraw your consent to its collection or sharing
- Request that we delete it, subject to limited legal exceptions
- Decline to have it sold (we do not sell it, and would never do so without your separate, signed authorization)
How to exercise your rights
To make a request, please use the contact form on this website. We will respond within the timeframe the law requires. If we decline a request, you may appeal by submitting another request through the contact form; if you remain unsatisfied after an appeal, you may contact the Washington State Attorney General's Office.
